NOTICE for all SBDIs!!!!!
Effective July 1st - All NPO's (Notice of Program Offering),
NPC's (Notice of Program Completion), Lost Certificate
Requests (Driver & Monitor/Attendant) should be mailed to
James Dundon, NYSED, Office of Educational Management
Services, Room 876 EBA, Albany, NY 12234. This is a
temporary move until our new contract for this program has
been approved.
COURSE LOCATOR Find a Driver or Monitor/Attendant Course in your area.
COURSE FORMS Course registration forms and course materials. Print
and complete by hand or fill out online and print.
Monitor PPT change. Only the last 4 digits of monitor or attendants' Social
Security Number need to be entered on the PT 910.
Effective immediately Monitor/Attendant NPC's do not need to include the
entire Social Security Number, JUST THE LAST FOUR DIGITS. This policy
is being implemented to protect against identity theft while continuing to
provide positive identification, especially for common names. Updated
forms may be downloaded and the procedure is effective immediately.
